About the New Mexico Early Childhood Transition Initiative
The mission of the New Mexico Early Childhood Transition Initiative is to support community teams in developing and/or improving their system of transitioning children and families from early intervention services to other services and supports.
Our vision is that all children and families will experience a smooth and effective transition as a result of collaborative intentional community planning.
Smooth Means:
- Uninterrupted Services
- Early intervention services on IFSP are available and provided until the first day of services by receiving agency
- IEP is in place at least 15 days prior to transition
- Services and supports begin on the date specified on the IEP
- Evidence of an interagency planning process which incorporates family centered practices
- Interagency agreements
- Mutually developed policies and procedures
- Formalized systems of communication and information sharing
Effective Means:
- Families informed of all options and all rights
- Timely Planning
- Transition planning initiated at least 6 months prior as evidenced by identified transition steps in the IFSP
- 90 Day Conference occurs and includes participation of Part B (if child eligible for Part B) and other relevant parties
- High Level of Satisfaction
- A demonstrated high level of satisfaction related to the experience of transitioning from one system to another as reported by the family, sending agency, and receiving agency/agencies.
