Our Goal
Our goal is for each student to develop and achieve a whole life plan
which is integrated, with the student’s active participation, into Transition Planning and achieved through student, family, community and school partnerships.

The first step is to discover the student’s current quality of life.

A trained self-advocate who has transitioned to adult life and a Community Guide interview the student to determine quality of life.

They’ve planted gardens, written poetry, created a rap song, gotten jobs, gone to the “Live with Kelly and Regis Show”,  gotten much needed community supports, traveled…
they’ve just begun!

In 2001-2002, 6 SFPS students completed Quality of Life Enhancement Interviews and
5 completed PATHs which were integrated into Transition Planning.
