Therapist's Letter of Medical Necessity/Justification

Your packet should include a letter of medical necessity/justification written by the Speech/Language Pathologist and as many other professional caregivers as possible. Some advocates believe that the more letters of medical necessity, the stronger the case.

Professional's Letter of Medical Necessity/Justification Outline:

Letter (s) could be written by:

  • Speech/Language Pathologist

  • Occupational Therapist

  • Physical Therapist

  • Nurse

  • Psychiatrist

  • Case Manager

  • Teacher (s)

  • Professional Caregivers

The letter should address the following as appropriate to the individual:

  1. Medical History

  2. Diagnosis

  3. Length of time for provided services

  4. Prognosis for speech

  5. Discuss the inability to adequately treat the client due to communication impairment - give examples.

  6. Discuss how a communication device would allow the client to receive maximum benefits from needed services

  7. Discuss your experience with trial periods on systems and your observations

  8. Recommended communication system by name with all necessary accessories

  9. Other personal supportive comments
