Your packet should include a letter of medical necessity/justification written
by the Speech/Language Pathologist and as many other professional caregivers as
possible. Some advocates believe that the more letters of medical necessity, the
stronger the case.

Discuss the inability to adequately treat the client due to communication

observations

Recommended communication system by name with all necessary accessories
