The Physician's Letter of Medical Necessity -
a Written Order

It is necessary for the client's primary physician to "prescribe" the augmentative communication system (including accessories) in order to receive funding from Medicaid or any other 3rd party payor. This prescription is often times written out on a standard prescription pad just like a prescription for oral medication, in other words, very short. It is recommended that the standard prescription be attached to a Physician's Letter of Medical Necessity. We are providing 2 sample letters. The samples are guides for you to follow, it will probably work best if you type the letter for the physician or provide him with a very specific sample for his/her use.

Physician's Letter/Outline

The letter should address the following key points:

  1. Medical History

  2. Diagnosis

  3. Prognosis (overall & speech/language)

  4. Inability of physician to adequately treat client due to communication impairment (specific examples are helpful)

  5. Recommended communication system deemed as medically necessary, list the specific equipment

  6. Other supportive comments including personnel observations
